Account Transfer 
This form is used when a customer needs to transfer money or positions from another firm to CCTG.

Canadian Non Solicit 
Required for Canadian Citizens.

Email Statement
Required for customers who wish to receive their daily and monthly statements via email.

Corporate Own Funds

Letter of Direction 
The letter of direction is a requirement for those customers who have a broker that will trade a third-party trading system.

Own Funds 
Required for customers that indicate their occupation may be investment related.

POA Forms 
Required for customer that indicate a third party will be trading the account on behalf of the customer. Customer will not be notified of the trades as they are placed.

Second Account Request Letter 
Required for customers that want more than want more than one account.

W-8BEN Certificate of Foreign Status 
The W-8BEN form is required for all foreign customers.
